For mail in  auditions and general registration please print, read and sign all forms and return to us as soon as possible.
Please send in a DVD, VHS,Flip Film or Teacher Recommendation Form.
Submit with $30.00 fee and pictures

We understand it may take some time to pull together all the Information required on these forms, so don’t panic.
If waiting for doctor’s signature please send in all other forms first.  Just be sure to have them in no later than May 25, 2012.
